It&#8217;s a macrocyclic peptide \u2014 a ring-shaped molecule \u2014 engineered to attach to PCSK9 and stop it from working. Merck markets it as LIPFENDRA\u00ae in 20 mg tablets.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Until this approval, patients who needed a PCSK9 inhibitor had to use injectable drugs like Repatha or Praluent. Lipfendra changes that by putting PCSK9 inhibition into pill form for the first time.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What Is Lipfendra Used For?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The FDA approved Lipfendra as an <strong>adjunct to diet and exercise<\/strong> to lower LDL cholesterol in adults with:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Hypercholesterolemia<\/strong> (high LDL cholesterol), including patients already on a statin who need further LDL lowering<\/li>\n\n\n\n<li><strong>Heterozygous familial hypercholesterolemia (HeFH)<\/strong>, an inherited condition that causes very high LDL from a young age<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">It is not a first-line treatment. Doctors typically prescribe it for patients whose LDL remains too high despite moderate- or high-intensity statin therapy, with or without other cholesterol drugs.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">How Does Lipfendra Work?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Lipfendra targets <strong>PCSK9<\/strong>, a protein your liver makes that normally breaks down LDL receptors. Fewer LDL receptors means your liver can pull less LDL cholesterol out of your bloodstream, so LDL levels rise.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Enlicitide binds to PCSK9 and blocks it from attaching to LDL receptors. A related <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/36889610\/\" target=\"_blank\" rel=\"noopener\">Phase 2b trial published in the peer-reviewed literature<\/a> found dose-dependent LDL-C reductions of up to roughly 61% at the highest dose tested.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\">&#8220;In two Phase 3 trials, LIPFENDRA led to impressive reductions in LDL-C.&#8221; \u2014 <strong>Dr. Ann Marie Navar<\/strong>, cardiologist at UT Southwestern Medical Center and lead investigator on the CORALreef Lipids trial<\/p>\n<\/blockquote>\n\n\n\n<p class=\"wp-block-paragraph\">It&#8217;s worth noting: LDL-lowering is a <strong>surrogate marker<\/strong>. A separate, larger trial \u2014 CORALreef Outcomes, enrolling more than 14,500 participants \u2014 is still underway to confirm whether Lipfendra actually reduces heart attacks and strokes, not just cholesterol numbers.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Lipfendra Side Effects<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">In the CORALreef Lipids trial, side effects with Lipfendra were similar to placebo overall. Effectiveness looks broadly comparable on paper, but head-to-head trial data between Lipfendra and the injectable drugs isn&#8217;t yet available.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">How Much Does Lipfendra Cost?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Merck has said it plans to price Lipfendra at roughly <strong>$10.50 per day<\/strong> for a one-month supply. As with most brand-name drugs, your actual out-of-pocket cost will depend on your insurance coverage and any manufacturer assistance programs.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">FAQs<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Is Lipfendra a statin?<\/strong> No. Statins work by blocking cholesterol production in the liver. Lipfendra works differently, by blocking the PCSK9 protein. It&#8217;s meant to be added to a statin, not to replace one, in most patients.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>How fast does Lipfendra lower cholesterol?<\/strong> Trial data measured LDL-C reduction at 24 weeks, but PCSK9 inhibitors typically start lowering LDL within the first few weeks of use. Your doctor will recheck your lipid panel to confirm your response.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Does Lipfendra prevent heart attacks?<\/strong> That hasn&#8217;t been proven yet.
